THRIVE ACADEMY: Waiver and Contract

As your hypnotherapist and your personal coach, I want to CONGRATULATE you for investing in yourself in a way that you may not have done before.

You'll see from what's included in this therapeutic experience that it is designed to deliver an extremely high level of accountability, loving support, and the right system, so you can make empowered decisions and take bold action toward achieving your goals. Saying "Yes" to this experience means showing up for yourself in a whole new way, mentally, energetically, and spiritually. This is the KEY to achieving everything you want. So is asking for support and being open to new ways of thinking. PLUS, being willing to be challenged to go outside your comfort zone will help you make your TOTAL TRANSFORMATION a reality that much faster.

In order for you to get the most out of your transformation, you agree to make the following commitments.

Your Commitment to the Program Includes

• Reading, signing, and returning this AGREEMENT prior to the first session.
• Showing up to your calls on time and ready for what we are going to do.
• Being authentic, honest, and speaking up for what you want.
• Letting me know about any personal situations that may interfere with your experience in this program.
• Letting it be okay to not know all the answers.
• Being willing to try new things and stretch beyond your current experience.
• Being responsible for your own results, which includes proactively asking for support, asking questions, staying committed to the process, and using your program's benefits.
• Consistently doing the work that is asked of you.
• Committing to yourself and the therapeutic process.
• Honoring the investment you are making in yourself by handling any situations that may take you away from your experience.
• Being patient with yourself and the process.
• Celebrating your wins, shifts, achievements, and excitement throughout the program.

Code of Conduct and ConfidentialityKindness and Respect

Participants are expected to foster a positive, supportive, and respectful environment for all members of the program. This includes:

• Speaking kindly and respectfully to other participants, facilitators, and guest speakers.
• Avoiding disruptive behavior or language that could harm or discourage others.
• Encouraging an inclusive and uplifting atmosphere during group sessions and interactions.

Confidentiality

To ensure a safe and trusting space for all participants, the following confidentiality guidelines apply:

• Any personal information, stories, or experiences shared during sessions must remain within the group and cannot be disclosed to individuals outside of the program.
• Participants agree not to record, share, or distribute any content from the sessions without prior written consent from all involved parties.
• Respect the privacy of all participants and facilitators by refraining from discussing their contributions outside of the program context.

Commitment to the Group

Participants acknowledge the importance of maintaining a respectful and confidential environment. Any violation of these guidelines may result in removal from the program at the facilitator’s discretion, without refund.

• Believing in you and your ability to achieve your vision of success.
• Sharing in-depth information and knowledge with you so that you can move forward with your goals much more quickly than you would on your own.
• Allowing you to be 100% authentic and FULLY YOU.
• Holding you accountable to act from your highest self, even when you may not feel that way.
• Being in integrity and honest at all times.
• Holding high standards for you and myself.
• Holding space for your progress so that you always know you have a safe place to express where you are.
• Sharing creative action steps, ideas, and inspiration without the need to have you "like" them but in the effort to give you options.
• Laser coaching you to find solutions to challenges that may come up.
• Coaching you to your highest self and to achieve your highest potential. Knowing that there are unlimited possibilities available to you, no matter what any particular situation looks like.
• Caring deeply about you and your success!

Program Investment

The investment for the Thrive Academy program will vary based on the payment option selected by the participant (e.g., full payment, payment plan) and any applicable discounts (e.g., early bird pricing).

The specific investment amount and payment terms will be outlined in the invoice provided upon registration. By signing this agreement, the participant agrees to the terms and payment schedule detailed in their invoice.

Payment Terms

Participants are responsible for completing payments according to the payment option selected at the time of sign-up. Payment plans will continue until all installments have been paid in full, even if the program concludes before the final payment is made.

For payment plans, monthly payments will be automatically deducted from the credit card provided at the time of sign-up. Failure to complete payments as agreed may result in removal from the program and/or legal action to recover outstanding balances.

Participants may choose from the following payment options for the 12-week program starting January 29th, 2025. This program, valued at over $11,000.00, is available at the following rates:

Commitment

The participant understands and agrees that once this agreement is signed, they are committing to the full investment amount outlined in their invoice, regardless of the payment option selected.

Non-Refundable Payments

All payments made are non-refundable, ensuring the participant’s commitment to their transformation and the program’s resources.

In the event of your absence or withdrawal, for any reason whatsoever, you will remain fully responsible for the entire program fee and any unpaid balance. This refund policy exists for your benefit as well. Coaching and hypnotherapy is a powerful process and, at times, you may feel "up against a wall" or as if something isn't working. Feeling uncomfortable is often a perfect sign that you are releasing obstacles and limiting beliefs.

My request is that any and all concerns regarding assignments and coaching be addressed directly with me, so I can be the best coach and mentor to you that I can be and ensure the coaching relationship is in integrity. Working together, we will make sure your needs are met!

Disclaimer

You (sometimes referred to herein as "Client") understand that the information received from me (sometimes referred to herein as "Coach" or "Hypnotherapist") in connection with the Program or otherwise should not be considered medical, nursing, or nutrition advice. It is not intended to replace consultation with licensed health professionals, including your doctor.

You understand and agree that:

• I am not providing health care, medical, or nutrition therapy services and will not diagnose, treat, or cure, in any manner whatsoever, any disease, condition, or other physical or mental ailment of the human body.
• I am not acting in the capacity of a doctor, licensed dietitian-nutritionist, massage therapist, psychologist, or other licensed or registered professional.
• You have voluntarily chosen to work with me and participate in the Program.

As your Hypnotherapist and Coach, I encourage you to maintain a relationship with your primary care physician or doctor. If you do not currently have one or do not undergo routine physical examinations, I recommend that you do so.

Important: Do not discontinue or alter any treatment plan or medical advice provided by your healthcare provider as a result of our sessions without first consulting them.

Release

You acknowledge and take full responsibility for your life and well-being, as well as the lives and well-being of your family (if applicable), and all decisions made during and after the Program.

In furtherance of this acknowledgment, you hereby and forever waive, release, and discharge me, my heirs, executors, administrators, assigns, officers, agents, employees, representatives, and all others acting on their behalf (collectively, the "Released Parties") from any and all claims or liabilities for injuries or damages to your person and/or property or that of your family (if applicable). This includes those caused by negligent acts or omissions of any of the Released Parties, arising out of or connected with your participation in the Program or in connection with services provided by me or the Released Parties.

Legal Items

• Modifications: This Agreement may not be modified without the prior written consent of both the Client and the Coach.
• Waivers: The waiver of any breach, right, or obligation shall not constitute a waiver of any other or subsequent breach, right, or obligation.
• Severability: If any provision of this Agreement is found to be invalid or unenforceable, the remainder of this Agreement shall remain in full force and effect.
• Entire Agreement: This Agreement constitutes the entire agreement between the parties and supersedes all prior proposals, agreements, and representations, whether written or oral.
• Assignment: Neither party may assign this Agreement without the prior written consent of the other party. This Agreement shall be binding upon and benefit the parties and their respective successors and permitted assigns.
• Survival: Provisions of this Agreement that by their nature and context are intended to survive the termination of this Agreement shall remain in effect.
• Jurisdiction: This Agreement consents to the exclusive personal jurisdiction of the Provincial and Federal Courts in Canada and acknowledges that venue is proper only in such courts.

Acknowledgment and Acceptance

If the terms of this Agreement are acceptable, please click the box in acknowledgment below. By signing, you confirm that:

• You have received a copy of this Agreement.
• You have had the opportunity to discuss its contents with me and, if desired, review it with an attorney.
• You understand, accept, and agree to abide by its terms.

IN WITNESS WHEREOF, the Client and Coach agree to the terms set forth and have duly executed this Agreement effective as of the Coach's signature date below.
